About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Hospice Care team as a Hospice Registered Nurse. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in their homes.

Key Responsibilities
  • Provide Direct Patient Care: Assess patient needs, develop and implement individualized care plans, and provide skilled nursing care to patients in their homes.
  • Collaborate with Interdisciplinary Team: Work closely with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive care and support for patients and their families.
  • Coordinate Care Services: Manage patient care services, including medication management, wound care, and palliative care, to ensure seamless delivery of care.
  • Communicate Effectively: Maintain open and honest communication with patients, families, and healthcare team members to ensure informed decision-making and optimal care outcomes.
  • Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes, to ensure continuity of care.
Requirements
  • Current RN License: Possess a current and active RN license in the state of employment.
  • Experience in Hospice Care: Have at least 1 year of experience in hospice care, home health, or a related field.
  • Strong Communication Skills: Possess excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
  • Compassionate and Empathetic: Demonstrate a compassionate and empathetic approach to patient care, with a focus on providing comfort, dignity, and respect to patients and their families.
