Role Overview:
The Inpatient Care Coordination Specialist (ICCS) is crucial in facilitating optimal recovery for patients. This role involves visiting skilled nursing facilities to conduct weekly assessments and collaborating with the interdisciplinary care team to ensure effective discharge planning. The ICCS engages with patients and their families to provide essential information and support informed decision-making, acting as a vital link between patients and healthcare professionals to guarantee smooth transitions of care.
Key Responsibilities:
- Serve as the primary liaison between patients and healthcare providers to ensure effective care transitions.
- Conduct assessments at Skilled Nursing Facilities (SNFs) utilizing clinical expertise and CMS criteria upon patient admission and throughout their stay.
- Review and discuss Length of Stay (LOS) targets, desired outcomes, and discharge plans with healthcare providers and families.
- Complete concurrent reviews for SNFs and manage authorizations promptly.
- Collaborate with the healthcare team, including physicians, discharge planners, and therapists, to facilitate optimal discharge outcomes.
- Monitor patient progress towards discharge goals and address any barriers to care.
- Participate in weekly SNF rounds, providing updates to management.
- Ensure appropriate referrals to health plans and community services are made.
- Engage with patients and families regularly, both on-site and via phone.
- Attend care conferences to discuss patient needs and progress.
- Assess the ongoing appropriateness of SNF placements based on CMS guidelines.
- Coordinate peer reviews with medical directors as necessary.
- Support the initiation of new contracts by collaborating with experienced staff.
- Manage a caseload effectively, utilizing strong time management skills.
- Document patient interactions and care plans accurately in the system.
- Review operational reports to identify areas for improvement.
- Adhere to all organizational policies and maintain confidentiality of patient information.
Qualifications:
- Active and unrestricted clinical license as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist.
- A minimum of 3 years of clinical experience in a relevant field.
- Proficiency in Microsoft Office Suite (Outlook, Excel, Word).
- Reliable transportation for travel to various facilities.
- A dedicated workspace at home for remote work.
- Access to high-speed internet.
Preferred Qualifications:
- Knowledge of CMS, Medicare, and Medicaid regulations.
- Experience in case management.
- Background in working with geriatric populations.
- Skills in patient education and rehabilitation.
- Strong attention to detail and goal-oriented mindset.
- Excellent communication and interpersonal skills.
- Proven problem-solving and conflict resolution abilities.
- Ability to prioritize and manage multiple tasks effectively.
