We are seeking a highly skilled and compassionate Patient Care Manager to join our team at Heartland Hospice. Key Responsibilities- Manage the clinical and administrative aspects of the hospice program, ensuring compliance with regulatory guidelines and company policies.
- Supervise and coordinate the work of nursing staff, including scheduling, training, and performance evaluations.
- Develop and implement strategies to improve patient outcomes, satisfaction, and quality of care.
- Collaborate with the Executive Director, Administrator, and Senior Patient Care Manager to achieve business objectives and goals.
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes as needed.
- Registered Nurse with a minimum of Associate's Degree in Nursing required from an accredited school of nursing.
- Three years' experience in direct patient care in a clinical setting required.
- Minimum one year of management/supervisory experience required.
- Two or more years' experience in hospice or home care required.
- Current license to practice nursing in the state of requested employment.
- CPR Certification.
- Current automobile insurance and valid driver's license.
- Bachelor's Degree in Nursing preferred from an accredited school of nursing.
- Two or more years' experience in hospice or home care preferred.
