Job Summary

We are seeking a highly skilled and compassionate Patient Care Manager to join our team at Heartland Hospice. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice site, ensuring the delivery of high-quality patient care, and managing a team of healthcare professionals.

Key Responsibilities
  • Manage the clinical and administrative aspects of the hospice program, ensuring compliance with regulatory guidelines and company policies.
  • Supervise and coordinate the work of nursing staff, including scheduling, training, and performance evaluations.
  • Develop and implement strategies to improve patient outcomes, satisfaction, and quality of care.
  • Collaborate with the Executive Director, Administrator, and Senior Patient Care Manager to achieve business objectives and goals.
  • Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes as needed.
Requirements
  • Registered Nurse with a minimum of Associate's Degree in Nursing required from an accredited school of nursing.
  • Three years' experience in direct patient care in a clinical setting required.
  • Minimum one year of management/supervisory experience required.
  • Two or more years' experience in hospice or home care required.
  • Current license to practice nursing in the state of requested employment.
  • CPR Certification.
  • Current automobile insurance and valid driver's license.
Preferred Qualifications
  • Bachelor's Degree in Nursing preferred from an accredited school of nursing.
  • Two or more years' experience in hospice or home care preferred.
