The Skilled Inpatient Care Coordinator (SICC) is essential in streamlining the recovery processes for patients. This role involves visiting skilled nursing facilities to conduct regular functional evaluations and collaborating with the interdisciplinary care team to facilitate effective discharge planning. The SICC engages with patients and their families to provide critical information and support informed decision-making. Acting as a liaison between patients and healthcare professionals, the SICC ensures that transitions of care are executed efficiently and effectively.

Key Responsibilities:

  • Serve as the primary connection between patients and healthcare providers to guarantee smooth transitions of care.
  • Conduct assessments at Skilled Nursing Facilities (SNFs) using clinical expertise and CMS criteria upon patient admission and throughout their stay.
  • Collaborate with healthcare teams to review Length of Stay (LOS) targets, desired outcomes, and discharge strategies.
  • Perform concurrent reviews at SNFs, ensuring timely updates to authorizations.
  • Work closely with physicians, discharge planners, and other healthcare professionals to optimize discharge processes.
  • Monitor patient progress towards discharge goals and address any obstacles encountered.
  • Participate in weekly SNF rounds, providing up-to-date information to senior management.
  • Ensure appropriate referrals to health plans and community services are made.
  • Engage with patients and families regularly, either through phone or in-person visits.
  • Attend care conferences to discuss patient needs and progress.
  • Assess ongoing appropriateness for SNF placement based on CMS guidelines.
  • Manage a caseload effectively, utilizing strong time management skills.
  • Maintain accurate documentation in the nH coordinate system.
  • Review monthly performance reports to identify areas for improvement.
  • Adhere to all organizational policies and maintain confidentiality of patient information.

Qualifications:

  • Active and unrestricted clinical license as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist.
  • A minimum of 3 years of clinical experience in a relevant field.
  • Proficiency in Microsoft Outlook, Excel, and Word.
  • Reliable transportation for travel to various facilities.
  • A dedicated workspace at home with high-speed internet access.

Preferred Qualifications:

  • Knowledge of CMS, Medicare, and Medicaid regulations.
  • Experience in case management and working with geriatric populations.
  • Background in patient education, rehabilitation, or home health nursing.
  • Strong communication, problem-solving, and decision-making skills.
