An experienced case coordinator, preferably bilingual, who possesses a genuine passion for supporting children and families with special healthcare needs. Prior experience in serving this demographic is highly desirable, although other relevant experience will also be considered.

  • Familiarity with community resources and the capability to connect families with essential services and support.
  • Skill in identifying issues and crafting effective, tailored solutions for families' needs.
  • Experience in managing multiple cases, maintaining precise records, and organizing documentation.
  • Ability to prioritize tasks and manage time efficiently to effectively serve multiple families.
  • Strong advocacy skills for the needs and rights of clients within various systems and institutions.
  • Emotional resilience and self-care practices to navigate the demands of working with families in challenging situations while maintaining personal well-being.
  • Capacity to collaborate effectively in a team environment and contribute positively to organizational culture.
  • Bachelor's degree in a relevant field.
  • A minimum of 2 years of related experience in case management or community outreach services within programs serving families with special healthcare needs.
  • Bilingual candidates are preferred.
  • Valid Texas Driver License.

Key Responsibilities

Your duties will include formulating comprehensive service plans that provide referrals and direct assistance tailored to the specific needs of each child and family. You will regularly review and update family service plans, ensuring that progress notes are current and adjustments are made as necessary. Staying informed about local, state, and national resources will be crucial in benefiting your clients.

Assist families in navigating and completing applications for insurance and funding programs, including but not limited to CSHCN, SSI, and Medicaid waivers. Implement evidence-based practices in service delivery, such as Person-Centered Planning, Trauma-Informed Care, and the CSHCN Practice Model. Manage client interactions, case files, and the ClientTrack database, ensuring timely updates, thorough documentation, and accurate information.

Conduct client interviews in private settings to ensure confidentiality. Safeguard sensitive information obtained during your work. Utilize pre-surveys to guide the development of service plans for active cases and post-surveys for cases pending deactivation.

Maintain an average client caseload as designated by program requirements. Engage in ongoing training, community networking, and agency courses related to resources available to families in need. Follow the approval process for any incurred expenses and contribute to the planning, coordination, and attendance of agency-sponsored events. Provide direct and indirect support to fundraising efforts and perform other duties, tasks, and special projects as assigned.
