Role and Responsibilities
The Case Management Team Lead is responsible for organizing, overseeing, and ensuring that all services provided by their case management team comply with contractual obligations and adhere to established guidelines. This role requires a high level of oversight in the execution and documentation of case-related activities. Utilizing advanced expertise in child welfare, the Case Management Team Lead will collaborate closely with the Case Management Director, offering insights and recommendations regarding the overall management and delivery of services to children and families within their team's caseload.
This position will involve coordinating and facilitating initiatives aimed at achieving permanency for children in care. The role encompasses a blend of administrative responsibilities, training, compliance monitoring, and fieldwork.
Key Duties
- Mentor and support case management services in accordance with external contracts and licensing regulations, including conducting and documenting weekly one-on-one meetings with team members.
- Coordinate admissions, case planning, and discharges of children and youth in compliance with relevant standards.
- Collaborate effectively with various professional agencies associated with Divinity Family Services.
- Serve as a liaison during licensing inspections and contract audits.
- Provide training and support for new staff, assist in work planning, and contribute to staff development and hiring processes.
- Promote and manage a culture that reflects the organization's mission and core values, fostering high engagement across all aspects of the organization.
- Review documentation for accuracy and ensure effective case planning and implementation of treatment plans.
- Assist case managers in ensuring all necessary documentation is collected and filed to maintain compliance.
- Meet compliance standards and deadlines as outlined by relevant regulations.
- Maintain open communication with the Case Management Director regarding any concerns or issues related to families and children.
- Provide statistical data to the Case Management Director for agency compliance on a weekly and monthly basis.
- Conduct field visits with team members for quality evaluations and additional visits as necessary.
- Monitor case managers' caseloads and provide recommendations for home assignments.
- Prepare performance reviews and assist in the development of corrective documentation as needed.
- Facilitate in-person team meetings bi-monthly at the direction of the Case Management Director.
- Provide office coverage as required.
- Approve leave requests and assist in creating coverage plans.
- Offer emergency and on-call services as needed.
- Maintain a manageable caseload as required.
- Adhere to the organization's leadership principles.
- Perform other duties as assigned.
Qualifications and Education Requirements
- Bachelor's degree in social work or a related field.
- Experience in the foster care system or familiarity with the Texas DFPS system is preferred.
- Knowledge of Minimum Standards for Child Placing Agencies is advantageous.
- Bilingual candidates are preferred.
- Leadership experience is preferred.
Conditions of Employment
Employees must possess and maintain a valid Texas driver's license. Background checks and random drug screenings may be conducted. Employees must comply with age requirements for full-time work in accordance with Texas Child Labor Law and the Federal Fair Labor Standards Act. Weekend and evening work may be necessary. Employees must maintain cellular phone service for on-call availability. Compliance with federal and state laws, as well as organizational policies regarding the confidentiality of healthcare information, is mandatory. Physical requirements may include standing, walking, sitting, using hands, reaching, climbing, and occasionally lifting up to 30 pounds.
