Position Summary:

The Bilingual Medical Case Manager (MCM) plays a crucial role in enhancing health outcomes for individuals living with HIV by delivering thorough case management services. This position focuses on crafting and executing personalized care strategies, ensuring seamless care coordination, and advocating for clients to facilitate access to essential medical and support resources. Medical Case Management encompasses a variety of client-centered initiatives aimed at improving health results within the HIV care continuum.

Key Responsibilities:

  • Deliver comprehensive case management services to all assigned clients.
  • Conduct client intakes, needs evaluations, and care plans upon first contact with new clients.
  • Maintain consistent communication with clients and perform ongoing needs assessments and care plans, aiming for client independence.
  • Manage applications and recertifications for ADAP/THMP programs and ensure accurate reporting.
  • Execute thorough client evaluations to identify healthcare needs and create customized care plans.
  • Coordinate and oversee medical appointments, treatment plans, and medication compliance.
  • Provide health education, counseling, and support to enhance treatment effectiveness.
  • Connect clients with vital community resources and services.
  • Keep precise and current client records, including electronic health documentation.
  • Collaborate with healthcare professionals and other stakeholders to ensure optimal service delivery.
  • Comply with program guidelines, regulations, and quality assurance standards.
  • Offer advocacy and support for clients as necessary.
  • Perform additional duties as assigned.

Qualifications & Skills:

  • Bilingual proficiency in English and Spanish is essential.
  • Strong organizational and time management capabilities.
  • Exceptional written and verbal communication skills.
  • Experience in case management, ideally within a healthcare environment.
  • Familiarity with electronic health records and data management systems.
  • Awareness of and sensitivity to issues related to HIV/AIDS, cultural diversity, LGBTQIA+ matters, substance use disorders, and grief.

Education, Licensure & Experience:

  • A Bachelor's degree in social work or a related field is mandatory.
  • A minimum of 1 year of experience or personal involvement with the LGBTQIA+ community is required.
  • A minimum of 1 year of experience or personal involvement with individuals living with HIV/AIDS or chronic health conditions is required.

Work Schedule:

  • Standard hours are Monday to Friday, 8 AM - 4:30 PM.
  • Flexible scheduling may be accommodated as needed.
  • Occasional travel may be necessary.

Work Environment:

  • Work is primarily conducted in a professional office setting, with direct/indirect interaction with patients and clients.
  • Some duties may require travel and outdoor work.
  • Physical demands include tasks such as sitting, walking, lifting, and standing for extended periods. Must be able to lift at least 25 pounds using safe lifting techniques.
