Position Summary:
The Bilingual Medical Case Manager (MCM) plays a crucial role in enhancing health outcomes for individuals living with HIV by delivering thorough case management services. This position entails the creation and execution of personalized care strategies, care coordination, and client advocacy to guarantee access to essential medical and support services. Medical Case Management encompasses a variety of client-focused activities aimed at improving health results within the HIV care continuum.
Key Responsibilities:
- Deliver comprehensive case management services to all assigned clients.
- Conduct client intakes, needs evaluations, and care strategies upon initial engagement with new clients.
- Maintain consistent communication with clients and perform ongoing needs assessments and care plans, focusing on client self-sufficiency.
- Oversee ADAP/THMP program applications, recertifications, and necessary reporting.
- Perform in-depth client evaluations to identify healthcare requirements and formulate customized care plans.
- Coordinate and oversee medical appointments, treatment regimens, and medication compliance.
- Provide health education, counseling, and support to enhance treatment outcomes.
- Connect clients with vital community resources and services.
- Keep precise and current client records, including electronic health documentation.
- Collaborate with healthcare professionals and other stakeholders to ensure effective care delivery.
- Comply with program guidelines, regulations, and quality assurance standards.
- Offer client advocacy and support as necessary.
- Perform additional duties as assigned.
Qualifications & Skills:
- Bilingual proficiency in English and Spanish.
- Strong organizational and time management capabilities.
- Exceptional written and verbal communication skills.
- Experience in case management, ideally within a healthcare context.
- Familiarity with electronic health records and data management systems.
- Awareness of and sensitivity to HIV/AIDS issues, cultural and ethnic matters, diverse populations, LGBTQIA+ concerns, substance use disorders, opioid use disorders, and grief and bereavement.
Education, Licensure & Experience:
- A Bachelor's degree in social work or a related field is required.
- A minimum of 1 year of working or lived experience with the LGBTQIA+ community is required.
- A minimum of 1 year of working or lived experience with individuals living with HIV/AIDS or chronic health conditions is required.
Work Schedule:
- Regular schedule Monday to Friday 8AM - 4:30PM.
- Flexible schedule, if required.
- Occasional travel may be necessary.
Work Location:
- One location, in-person.
- Off-site work may be required as necessary.

Requirements:
