Job Summary and Qualifications
The Case Coordinator will collaborate with patients and families identified as needing discharge planning, psychosocial support, financial assistance, or other services to create a comprehensive plan that will:
1) Enhance functional and self-care capabilities;
2) Facilitate access to appropriate services;
3) Promote communication and coordination among healthcare team members;
4) Encourage quality, cost-effective outcomes.

The Case Coordinator will exhibit the knowledge and skills required to develop a plan based on physical, psychosocial, educational, and age-related criteria.

Key Responsibilities:
- Assess the discharge needs of high-risk patients through interviews and chart reviews. Identify available resources to address these needs, coordinate referrals, and document the plan.
- Utilize community resources to support patients' post-hospital needs and document the options provided.
- Monitor resource statistics to identify opportunities for improvement. Participate in medical staff and hospital committee meetings to share findings.
- Stay informed about requirements from JCAHO, HCFA, and other state/federal agencies regarding care provision, planning, and documentation.
- Facilitate and provide education to hospital staff, physicians, patients, and caregivers to ensure efficient resource utilization.
- Conduct admission and continued stay reviews, providing clinical information to payers upon request for care certification and reimbursement. Discuss alternative levels of care with physicians and refer cases not meeting acute care criteria to the Physician Advisor as necessary.

Qualifications:
Education:
Graduate of an accredited diploma, associate, or baccalaureate degree nursing program.

Experience:
A minimum of 3 years of clinical experience in the relevant area of Case Management.

Required:
Active RN License to practice as a professional registered nurse in the state of Texas or an Active Multi-State RN Compact License (those with an RN Compact License must obtain a Texas RN License within 90 days of hire).

Preferred:
Bachelor of Science in Nursing and at least one year of experience in utilization review, resource management, discharge planning, or case management.
