Program Summary:

California Advancing and Innovating Medi-Cal (CalAIM) is a long-term commitment to transform and strengthen Medi-Cal, offering Californians a more equitable, coordinated, and person-centered approach to maximizing their health and life trajectory. The program provides engagement, assessment, housing and community coordination support to transitional age youth (TAY) (ages 18-24).

Community Supports services through Cal-AIM includes:

Program participants who are experiencing homelessness or at risk of experiencing homelessness. receive help to find, apply for, and secure housing. Additionally, participants receive support to maintain safe and stable tenancy once housing is secured, such as coordination with landlords to address issues, assistance with the annual housing recertification process, and linkage to community resources to prevent eviction.

Position Summary:

CalAIM Housing Specialists - The Housing Specialist assists and advocates on behalf of participants to locate, secure, and retain affordable permanent housing. The Housing Specialist plans and coordinates services, identifies appropriate housing options, and interacts with landlords and other organizations on behalf of participants.

Requirements

Major Areas of Responsibility:
  • Provide trauma informed, strength-based, person centered case management services that include safety planning, advocacy, relationship building and empowerment
  • Screen participants through identified portals to ensure eligibility.
  • Complete Housing Assessments that identify participants' preferences and barriers related to housing.
  • Developing an Individualized Housing Support Plan (based on the Housing Assessment) that addresses identified barriers, short- and long-term housing goals, and identifies when other services may be required to meet housing goals
  • Assisting in securing housing, including the completion of housing applications and securing vital documents (e.g., Social Security, Birth Certificate, etc.)
  • Assisting with benefits advocacy, including assistance with obtaining identification and documentation for SSI eligibility and supporting the SSI application process
  • Identifying and securing available resources to assist with subsidizing rent (such as Section 8, state, and local housing assistance programs) and matching available rental subsidy resources to participants
  • If included in the Individualized Housing Support Plan, identifying and securing resources to cover expenses, such as security deposit, moving costs, adaptive aids, environmental modifications, moving costs, and other one-time expenses
  • Communicating and advocating on behalf of participants with landlords
  • Identifying and coordinating non-emergency, non-medical transportation to assist participants' mobility to ensure reasonable accommodations and access to housing options prior to transition and on move-in day
  • Providing early identification and intervention for behaviors that may jeopardize housing, such as late rental payment, hoarding, substance use, among other lease violations
  • Education and training on the role, rights, and responsibilities of the tenant-landlord relationship
  • Coaching Participants on developing and maintaining key relationships with landlords/property managers with a goal of fostering successful tenancy
  • Coordination with the landlord to address identified issues that could impact tenancy
  • Assistance in resolving disputes with landlords and/or neighbors to reduce risk of eviction or other adverse action, including developing a repayment plan or identifying funding in situations in which the participant owes unpaid rent or payment for damage to the unit
  • Coordinating with participants to review, update and modify their Individualized Housing Support Plan on a regular basis to reflect current needs and address existing or recurring housing retention barriers
  • Continuing assistance with lease compliance, including ongoing support with activities related to household management
  • Providing independent living and life skills including assistance with and training on budgeting, including financial literacy and connection to community resources
  • Collect and track data as needed for reporting and meeting project goals and objectives.
  • Maintain accurate client files, good recordkeeping, update as needed and adhere to confidentially policies.
  • Maintain current knowledge of CalOptima Health CalAIM rules and regulations.
  • Maintains confidentiality of client files per HIPAA and all applicable guidelines.
Experience Requirements:
  • Effective communication skills including writing, public speaking, and conflict resolution/mediation skills.
  • Highly motivated, willing to learn, and able to work independently at times.
  • Be comfortable with and understand young adult, adolescent & child abuse issues.
  • Proficient in Microsoft Office applications: Outlook, Word, Excel, Power Point.
Knowledge & Skills:
  • Excellent verbal and written communication skills.
  • Demonstrate an understanding of Trauma Informed Care, challenges/barriers with TAY population, resources available within Orange County community.
  • Ability, desire, and flexibility to work in a high crisis, fast paced environment.
  • Ability to multi-task with the capacity to learn quickly and integrate knowledge efficiently
  • Proficient in Microsoft Office: Outlook, Word, and Access. Preferred knowledge in data entry.
  • Ability to build internal and external working relationships
  • Exercises sound judgement
  • Exceptional organizational skills and attention to details.

Working Environment/Job Requirements:

  • Position will require use of personal vehicle.
  • Physical requirements include standing, sitting, typing, bending, and lifting up to approximately 50 lbs. (i.e. tables, chairs, food, water, etc.).
  • Possess a valid California driver's license and proof of automobile insurance.
  • Position requires DMV clearance.
