Position Summary:

The Outreach Specialist plays a crucial role in enhancing program visibility within the community, providing essential support to clients facing crises, and facilitating their journey towards stability and success. This position involves active community engagement to cultivate resources that benefit clients and their families.

Key Responsibilities:

  • Conducts outreach activities, providing crisis intervention, trauma-informed case management, and referral services to clients and their families.
  • Performs initial assessments and immediate needs evaluations to ensure timely support.
  • Collaborates with the case management team to deliver information referrals, crisis assistance, and aid in securing permanent housing for participants.
  • Assists in comprehensive assessments to gather vital information for developing tailored Individual Service Plans.
  • Coordinates follow-up services and maintains accurate records of all assistance provided.
  • May undertake responsibilities of a Case Manager or Housing Specialist as required.
  • Supports program staff in implementing development strategies and evidence-based practices to deliver holistic services, including life skills training.
  • Facilitates workshops addressing household needs and provides crisis intervention as necessary.
  • Leads community outreach initiatives, representing the organization at relevant meetings and fostering partnerships with local stakeholders.
  • Performs additional duties as assigned.

Qualifications:

Requirements:

  • Successful completion of a fingerprint clearance and background check, including criminal history and reference verification.

Education:

  • Bachelor's Degree in Social Services or a related field, or an equivalent combination of education and experience (four years).

Experience:

  • A minimum of two years working with justice-involved populations, individuals in recovery, or those experiencing homelessness.

Preferred Qualifications:

  • Bilingual proficiency in Spanish.
  • Familiarity with HMIS (Homeless Management Information System).
