Responsibilities
  • Works independently and in collaboration with the director of clinical operations in onboarding, training and managing the Chronic Care Management and Palliative Care program .
  • Supervises RNs, community health workers and social workers .
  • P rovides ongoing coaching , mentoring , audits and performance reviews.
  • Works with leadership and talent acquisition to recruit and hire p alliative c are providers.
  • Performs other duties as assigned , including patient visits.
Qualifications
  • Bachelor of Science in nursing.
  • Current RN license, specific to the state(s) you are assigned to work.
  • Three years of RN experience in pall iative care or geriatrics.
  • Two years of clinical management experience.
  • Competence in management of problems experienced by patients with life-limiting illness.
Preferred
  • Current CPR and ACLS certification.
