Palliative Advanced Practice Provider (PRN) - Hospital

Location: Nashville, TN and surrounding areas
Status: PRN (Float)
Days: Monday through Friday

We are seeking a Nurse Practitioner (NP) to serve as an inpatient Palliative Care consultant within local area hospitals to provide excellent patient care through optimized symptom management and complex discussions surrounding goals of care or advance care planning. Hospice or palliative care experience is strongly suggested, as well as a desire to independently build a strong referral base within the assigned facility.


SUMMARY

The NP provides advanced practice nursing care and/or consultation in the home, in an inpatient hospice facility, in an inpatient hospital or a long-term care facility. The NP functions as a member of the medical care management team.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Under the direction of the CMO or designee, diagnoses medical condition, plans and implements interventions, and evaluates patient response.
  2. Assembles and documents complete patient medical database including health history and physical examination.
  3. Assesses eligibility for palliative care and hospice services.
  4. Documents assessments, orders, interventions, results and discharge planning in the medical record.
  5. Communicates findings with consulting physicians and IDT members.
  6. Participates in IDT meetings as required.
  7. Provides information, education, and support to patients and families.
  8. Fosters teamwork in the IDT.
  9. Facilitates communication between patients, family members, and the IDT.
  10. Serves as a resource person to IDT.
  11. Promotes and practices economical and effective palliative care.
  12. Exhibits behaviors that reflect commitment to the philosophy and vision of Alive Hospice and Alive Palliative Care Services.
  13. Participates in medical and nursing research projects, including the presentation and publication of findings.
  14. Remains current on research findings and integrates into practice.
  15. Serves as a mentor and preceptor to nursing students, medical students, medical residents and fellows.
  16. Assists in orienting new Inpatient Unit staff to hospice and palliative care.
  17. Promotes excellence in nursing practice.
  18. Collaborates with Chief Operating Officer or designee to identify the need for and conduct in-services for clinical staff.
  19. Maintains NP certification and demonstrates continued competency in the management of patient care.
  20. Participates in professional organizations and serves on agency committees.
  21. Participates in after-hours call responsibilities.
  22. Other duties may also be assigned.

SUPERVISORY RESPONSIBILITIES

The NP does not directly supervise any employees but does provide feedback. The NP reports to the CMO, with input from the COO.


CONTINUING EDUCATION

The agency requires this position is required to complete 15 hours of continuing education per year covering topics that will contribute to improvements in carrying out the above responsibilities. Regulatory agencies may require some disciplines to have additional hours in order to be licensed or certified.

EDUCATION and/or EXPERIENCE

Registered Nurse with a current Tennessee nursing license. Minimum of three years nursing experience preferred. Master’s Degree in Nursing required.


CERTIFICATES, LICENSES, REGISTRATIONS

Current unrestricted Tennessee RN License. Nurse practitioner (ANP, FNP, or GNP) certification in Tennessee. If required to drive to carry out the duties of this position: current driver’s license and automobile insurance as required by Tennessee State Law.

The employee is occasionally exposed to fumes or airborne particles toxic or caustic chemicals, outside weather conditions, risk of radiation, and vibration. The noise level in the work environment is usually moderate. Patient’s home or impatient hospice facility is the norm.


Requirements:

Requirements

• Licensed Advanced Practice Provider in the State of Tennessee

• Certified as an Advanced Practice Provider by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).

• Medicare billing privileges or eligibility for privileges

• Have insurance required by state law to practice and prescribe in the state where services are provided, or be eligible for such insurance requirements to be satisfied by group coverage with company

• Current driver's license and automobile insurance as required by Tennessee State Law

• One (1+) year experience as a Nurse Practitioner. Three (3) to five (5) years total nursing experience.

Preferred

• Hospice, palliative, and/or geriatric practice experience

• Board Certification as an Advanced Certified Hospice and Palliative Nurse (ACHPN)

• Inpatient/Hospital-based experience as an NP or RN ICU experience

• Experience in developing a referral-based census

• Knowledge of disease-specific criteria for hospice eligibility
