We are seeking a compassionate and experienced RN to take on the role of Clinical Manager in our Palliative Care division. This position is ideal for a professional who is passionate about enhancing the quality of life for patients receiving care at home.

Key Responsibilities:
  • Collaborate with the Director of Clinical Operations to onboard, train, and oversee the Chronic Care Management and Palliative Care programs.
  • Supervise a team of RNs, community health workers, and social workers.
  • Provide ongoing coaching, mentoring, and performance evaluations.
  • Work with leadership and talent acquisition to recruit and hire palliative care professionals.
  • Perform additional duties as assigned, including patient visits.
Qualifications:
  • Bachelor of Science in Nursing.
  • Current RN license specific to the state(s) of employment.
  • A minimum of three years of RN experience in palliative care or geriatrics.
  • At least two years of clinical management experience.
  • Proficiency in managing the complexities faced by patients with life-limiting illnesses.
Preferred Qualifications:
  • Current CPR and ACLS certification.
