At Alive Hospice, Inc., we are dedicated to providing exceptional care and support to patients and their families during challenging times. We are looking for a compassionate Nurse Practitioner to join our team as an inpatient Palliative Care consultant.


Key Responsibilities:

The Nurse Practitioner will operate autonomously while collaborating with the supervising physician and the hospital's medical team to deliver outstanding palliative care. Responsibilities include:

  • Managing symptoms associated with both acute and chronic illnesses.
  • Facilitating communication and care coordination among the healthcare team.
  • Developing comprehensive treatment plans tailored to individual patient needs.
  • Working closely with patients and families to provide education on disease processes and establish care goals.
  • Exhibiting professionalism and excellent customer service to all patients and referral sources.

Qualifications:

To be considered for this role, candidates must meet the following requirements:

  • Valid Advanced Practice Provider license in Tennessee.
  • Certification as an Advanced Practice Provider by AANP or ANCC.
  • Eligibility for Medicare billing privileges.
  • Insurance coverage as mandated by state law.
  • Current driver's license and automobile insurance as required by state law.
  • A minimum of one year of experience as a Nurse Practitioner, with three to five years of total nursing experience.

Preferred Qualifications:

While not mandatory, the following qualifications are preferred:

  • Experience in hospice, palliative, or geriatric care.
  • Board Certification as an Advanced Certified Hospice and Palliative Nurse (ACHPN).
  • Experience in inpatient or hospital-based settings.
  • Proven ability to develop a referral-based patient census.
  • Knowledge of hospice eligibility criteria.
