We are looking for a dedicated Nurse Practitioner (NP) to act as an inpatient consultant in Palliative Care across various hospitals in the local area. The role focuses on delivering exceptional patient care through effective symptom management and facilitating complex discussions regarding care objectives and advance care planning. Experience in hospice or palliative care is highly recommended, along with a commitment to independently cultivate a robust referral network within the designated facility.

POSITION SUMMARY

The NP will provide advanced nursing care and/or consultation in various settings, including home care, inpatient hospice facilities, hospitals, or long-term care environments. The NP will collaborate as part of the medical care management team.

KEY RESPONSIBILITIES

  1. Diagnose medical conditions, plan and implement interventions, and assess patient responses under the guidance of the Chief Medical Officer (CMO) or their designee.
  2. Compile and document comprehensive patient medical records, including health history and physical assessments.
  3. Evaluate eligibility for palliative care and hospice services.
  4. Record assessments, orders, interventions, outcomes, and discharge plans in the medical documentation.
  5. Communicate findings with consulting physicians and interdisciplinary team (IDT) members.
  6. Engage in IDT meetings as necessary.
  7. Provide education, information, and support to patients and their families.
  8. Encourage teamwork within the IDT.
  9. Facilitate communication among patients, family members, and the IDT.
  10. Act as a resource for the IDT.
  11. Promote and implement effective and economical palliative care practices.
  12. Demonstrate behaviors that align with the philosophy and vision of Alive Hospice and Alive Palliative Care Services.
  13. Engage in medical and nursing research initiatives, including presenting and publishing findings.
  14. Stay updated on research developments and integrate them into practice.
  15. Serve as a mentor and preceptor for nursing students, medical students, residents, and fellows.
  16. Assist in orienting new staff in the Inpatient Unit to hospice and palliative care.
  17. Promote excellence in nursing practice.
  18. Collaborate with the Chief Operating Officer (COO) or their designee to identify training needs and conduct in-services for clinical staff.
  19. Maintain NP certification and demonstrate ongoing competency in patient care management.
  20. Participate in professional organizations and contribute to agency committees.
  21. Be available for after-hours call responsibilities.
  22. Other duties may be assigned as necessary.

SUPERVISORY ROLE

The NP does not directly supervise any employees but provides feedback and reports to the CMO, with input from the COO.

CONTINUING EDUCATION REQUIREMENTS

This position requires the completion of 15 hours of continuing education annually, focusing on topics that enhance the ability to fulfill the responsibilities outlined above. Regulatory agencies may mandate additional hours for specific disciplines to maintain licensure or certification.

EDUCATION AND EXPERIENCE

Registered Nurse with a valid Tennessee nursing license. A minimum of three years of nursing experience is preferred. A Master's Degree in Nursing is required.

CERTIFICATIONS AND LICENSES

Current unrestricted Tennessee RN License. Nurse practitioner certification (ANP, FNP, or GNP) in Tennessee. If driving is necessary for job duties, a valid driver's license and automobile insurance as mandated by Tennessee State Law are required.

PHYSICAL REQUIREMENTS

Driving an automobile is frequently required.

ADDITIONAL REQUIREMENTS


• Licensed Advanced Practice Provider in the State of Tennessee

• Certification as an Advanced Practice Provider by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)

• Medicare billing privileges or eligibility for such privileges

• Insurance as required by state law to practice and prescribe in the state where services are provided, or eligibility for group coverage through the company

• Current driver's license and automobile insurance as required by Tennessee State Law

• One (1+) year of experience as a Nurse Practitioner, with three (3) to five (5) years of total nursing experience.

PREFERRED QUALIFICATIONS


• Experience in hospice, palliative, and/or geriatric care

• Board Certification as an Advanced Certified Hospice and Palliative Nurse (ACHPN)

• Inpatient or hospital-based experience as an NP or RN in an ICU setting

• Proven experience in developing a referral-based patient census

• Knowledge of disease-specific criteria for hospice eligibility.
