Key Responsibilities

  • Collaborate with the director of clinical operations to onboard, train, and oversee the Chronic Care Management and Palliative Care initiatives.
  • Supervise a team of RNs, community health workers, and social workers.
  • Provide continuous coaching, mentoring, performance evaluations, and audits.
  • Partner with leadership and recruitment teams to attract and hire palliative care professionals.
  • Fulfill additional duties as required, including patient visits.

Qualifications

  • Bachelor of Science in Nursing.
  • Active RN license in the state(s) of practice.
  • A minimum of three years of RN experience in palliative care or geriatrics.
  • At least two years of experience in clinical management.
  • Proficient in managing issues faced by patients with life-limiting conditions.

Preferred Qualifications

  • Current CPR and ACLS certifications.
