Job Overview

POSITION SUMMARY

The Palliative Care Program Supervisor will oversee and support a segment of the palliative care initiative, specifically providing direct assistance to advanced practice providers while reporting to the Senior Director of Hospice & Palliative Services. This role will also manage a palliative care caseload and fill in for Advanced Practice Providers (APP) as needed to accommodate patient care demands based on the program's prioritized requirements.

KEY RESPONSIBILITIES

Administrative and Leadership Responsibilities:

  • Facilitate the establishment and development of team culture, ensuring high job satisfaction and support for APPs within the Palliative Care team to enhance team performance.
  • Conduct weekly check-ins with each team member, sharing insights with the Senior Director of Hospice & Palliative Services for support, education, feedback, coordination, or troubleshooting.
  • Provide coaching, support, and constructive feedback for performance enhancement for both seasoned and novice providers, covering areas such as:
    • Direct patient care, consultation protocols, documentation, mentorship, professional growth, and recommended continuing education units (CEUs).
  • Oversee scheduling in designated areas and manage on-call schedules as required.
  • Coordinate surge capacity needs, including the allocation of Float and PRN providers to ensure adequate coverage while being financially mindful.
  • Oversee the onboarding process for new APPs, including:
    • Developing and managing the onboarding schedule and its implementation.
    • Ensuring all logistical requirements are met.
    • Monitoring APP performance and improvement through attentive oversight and guidance.
    • Serving as the primary contact for questions, concerns, and troubleshooting.
  • Provide insights for performance evaluations of the palliative care team in collaboration with the Senior Director and Medical Director of Palliative Care.
  • Support the enhancement of Palliative Care understanding within the Alive organization.
  • Collaborate effectively with corporate support departments (IT, HR, Revenue Cycle).
  • Solicit and offer feedback to Palliative Care leadership, identifying areas for improvement through regular communication with the Senior Director.
  • Occasionally act as a public representative and resource for Palliative Care in the community, including conducting educational sessions.
  • Offer additional leadership and oversight during the absence of the Senior Director.
  • Perform other duties as assigned based on the program's needs.

EDUCATION AND EXPERIENCE REQUIREMENTS

A Master's Degree in a healthcare-related field is required; leadership experience is preferred. An Advanced Practice Provider designation is necessary, with board certification in Hospice & Palliative Care being advantageous. A proactive approach and positive collaboration with leadership are essential to optimize the Palliative Care program.
