What You'll Do:

As a Clinical Evaluator, you will be responsible for conducting thorough assessments of residents' health conditions, functional abilities, and care requirements. Your evaluations will drive the development of personalized care plans, ensuring the highest standard of care and well-being of our residents.

Key Responsibilities:

  • Perform comprehensive assessments of physical, mental, and emotional health to determine appropriate care setting
  • Evaluate functional abilities and care needs through detailed observations and assessments
  • Collaborate with interdisciplinary teams to develop individualized care plans
  • Develop and maintain positive relationships with area hospitals and community agencies
  • Establish and maintain relationships with social workers, case managers and other contacts in the healthcare and payor community
  • Educate residents and families about facility services, procedures, and care expectations
  • Monitor and reassess residents' progress and update care plans accordingly
  • Advocate for residents' well-being and coordinate with healthcare providers to address needs
  • Document assessments and care plans accurately and timely

What You'll Bring:

Qualifications of a Clinical Evaluator include:

  • Nursing or Social Work degree, with valid state license or certification preferred
  • 3 years of clinical or case management experience in long-term care
  • Experience in discharge planning and marketing activity
  • Compassionate and empathetic approach to patient care Interest in the nursing needs of the aged and the chronically ill with the ability to work with both
  • Strong clinical skills and critical thinking abilities Excellent communication and interpersonal skills
  • Ability to work effectively in a multidisciplinary team environment
