DEFINITION: The position of Program Support Coordinator is a responsible position requiring self-motivation, self-initiative, and independent decision making. The Program Support Coordinator will be cross-trained and available to train and perform Home Coordinator duties in all Woodhaven residential homes and assist with Program Management duties when needed. The responsibilities shall include supervising staff, coordinating client medical care and house emergency procedures, assuring that IP's are implemented appropriately, and managing day to day operations of the home. The Program Support Coordinator will be responsible to see that the home or homes is operating in accordance with the certification principles of the Department of Mental Health in accordance with CARF accreditation standards and that of Woodhaven Learning Center.


ESSENTIAL QUALIFICATIONS/FUNCTIONS OF THE POSITION: A high school diploma or equivalent is required and a degree in human services or supervisory and experience working with persons with developmental disabilities is preferred. This position requires that you successfully complete an initial orientation. This will include CPR, First Aid, Positive Behavior Interventions, Universal Precautions in-service, lifting in-service, a medication administration course, and other lectures on basic information regarding State and Federal law relevant to the organization's policies, procedures, rules, and regulations. You will be required to update these in-services annually or as required. A valid Missouri chauffeur's license within the first two (2) weeks of employment is necessary for this position and must maintain acceptable driving status according to Procedure No. 1-8-3, Woodhaven Vehicle Use. Any exception to this requirement must be authorized by the Chief Operating Officer or the Director of Residential and Community Services. Applicants must have the ability to cooperate and work well with the individuals receiving services, their families, and staff. This position also requires that you successfully complete the Management Training Curriculum course. All Home Coordinators who have committed felonies that are listed on the disqualification registry with the Department of Mental Health will be removed from payroll effective immediately unless a request for exception has been approved with the Department of Mental Health

Requirements:

Essential Qualifications/Functions of the Position also include: • Attendance/Ability to work scheduled time. Employees are expected to be reliable when reporting to work for scheduled shifts. • Must minimize overtime usage. • Ability to work prescribed number of hours • Acceptance of supervision • Ability to lift a minimum of 50 pounds • Chauffeur’s license • Willing and able to carry a cell phone and respond to calls 24 hours per day, seven days a week. • Provide support to HC 1 & 2’s during absence, scheduled vacations and vacant positions. • Provide on-call relief to team members who are scheduled off. • Weekend coverage is required with one guaranteed weekend off per month.


ESSENTIAL PHYSICAL REQUIREMENTS: This is sedentary work requiring the exertion of up to 50 pounds of force occasionally and a negligible amount of force frequently or constantly to move objects; work requires fingering, grasping, and repetitive motions; vocal communication is required for expressing or exchanging ideas by means of the spoken word; hearing is required to perceive information at normal spoken word levels; visual acuity is required for preparing and analyzing written or computer data, operation of machines, determining the accuracy and thoroughness of work, and observing general surroundings and activities; the worker is not subject to adverse environmental conditions.
