Job Overview

POSITION SUMMARY:
The role of the Program Support Coordinator is a vital position that demands a high level of self-motivation, initiative, and the ability to make independent decisions.

The Program Support Coordinator will be trained across various functions and will assist in fulfilling Home Coordinator responsibilities across all residential facilities, as well as supporting Program Management tasks as necessary.

KEY RESPONSIBILITIES:
The Coordinator will oversee staff, manage client medical care, and ensure emergency protocols are effectively implemented. This role also involves ensuring that Individual Plans (IP's) are executed correctly and managing the daily operations of the residential home.

The Program Support Coordinator is accountable for ensuring that the residential facilities operate in compliance with the certification standards set forth by the Department of Mental Health, adhering to CARF accreditation criteria, and maintaining the standards of Woodhaven Learning Center.

QUALIFICATIONS:
A high school diploma or equivalent is mandatory, while a degree in human services or relevant supervisory experience with individuals with developmental disabilities is preferred. Successful completion of an initial orientation is required, which includes training in CPR, First Aid, Positive Behavior Interventions, Universal Precautions, lifting techniques, medication administration, and relevant state and federal laws.

It is essential to obtain a valid chauffeur's license within the first two weeks of employment and maintain a satisfactory driving record as per company policy. Exceptions to this requirement must be approved by the Chief Operating Officer or the Director of Residential and Community Services.

The candidate must demonstrate the ability to collaborate effectively with clients, their families, and staff members. Additionally, completion of the Management Training Curriculum course is necessary. Home Coordinators with felony convictions listed on the disqualification registry will be removed from payroll unless an exception is granted by the Department of Mental Health.

ADDITIONAL REQUIREMENTS:
Essential qualifications and responsibilities include:

  • Consistent attendance and reliability in reporting for scheduled shifts.
  • Minimizing overtime usage.
  • Ability to work the required number of hours.
  • Acceptance of supervision and guidance.
  • Capability to lift a minimum of 50 pounds.
  • Possession of a valid chauffeur's license.
  • Availability to carry a cell phone and respond to calls 24/7.
  • Providing support to Home Coordinators during their absences and filling vacant positions.
  • Offering on-call relief to team members as needed.
  • Weekend coverage is mandatory, with one guaranteed weekend off each month.

PHYSICAL DEMANDS:
This position involves sedentary work requiring the ability to exert up to 50 pounds of force occasionally and a negligible amount of force frequently or constantly to move objects. Tasks require manual dexterity, vocal communication, and the ability to perceive information at normal spoken word levels. Visual acuity is necessary for preparing and analyzing written or computer data, operating machinery, and observing surroundings.
