Job DescriptionJob Description

The mission of Bridges to Care and Recovery is to mobilize clusters of churches in the African American communities of North St. Louis City and County to support the behavioral health treatment and recovery of congregants and other community members with behavioral health disorders. This is part of an overall mission of extending the system of care for people challenged with behavioral health disorders. The Outreach Coordinator is responsible for acting as a liaison between the faith-based community and community mental health services to provide linkage/referral services. The Outreach Coordinator will serve as the point of contact for congregations, the community and providers regarding services to include outreach activity, referrals to service providers, and coordination with providers regarding capacity for referrals.

Essential Duties and Responsibilities

  • Works with the faith-based community to provide education, referral information, and short-term case coordination.
  • Recruits churches to participate in the BCR training program and builds relationships with participating churches.
  • Fosters relationships with community agencies and grassroots organizations to schedule BCR training.
  • Actively participates in screening events to provide information about the referral line to congregants.
  • Promotes understanding of mental health conditions, healthy lifestyles, and other health-related information.
  • Provides community outreach services and serves as an advocate/referral source to help people utilize community resources.
  • Documents all referrals, provides initial contact within 48 hours, and ensures follow-up visits to troubleshoot access issues.
  • Ensures all churches are aware of the referral line and distributes BCR marketing materials to local organizations and churches.

Work Schedule: This position requires a flexible schedule. It should be noted that some evening and weekend hours will be required due to working closely with the faith-based community. Additionally, the applicant must be comfortable working in the communities served to include home and church visits.

Qualification, Skills and Knowledge Requirements

Qualifications:

  • Education: Bachelor’s degree in social services preferred. Individuals with 4 years of experience in a similar role working with individuals in the community will also be considered.
  • Experience: Prior experience providing case management and documentation services, preferably with individuals with mental health and/or co-occurring disorders.

Additional Qualifications:

  • Excellent written and oral communication skills.
  • Strong computer skills, particularly in Microsoft PowerPoint, Word, and Excel.
  • Strong group training and facilitation skills.
  • Strong customer service sensibility required.
  • Must be organized and able to meet documentation deadlines.
