  • Evaluating potential service areas to identify target demographics.
  • Engaging with healthcare providers, including physicians and facilities, to present our services and secure patient referrals.
  • Conducting thorough assessments of our organizational strengths and weaknesses.
  • Reviewing historical and current marketing data to inform strategies.
  • Creating and executing a comprehensive marketing plan with defined priorities and timelines.
  • Coordinating referrals across all Cedarhurst Senior Living services.
  • Participating in interdisciplinary group meetings as necessary.
  • Facilitating initial informational sessions for patients and their families.
  • Providing community outreach and educational initiatives.
  • Contributing to discharge planning efforts as required.
  • Assessing the emotional and social needs of patients and their families.
  • Consulting with healthcare team members to address various factors impacting patient health.

Qualifications for the Clinical Liaison position include:

  • A minimum of one year of experience in marketing, nursing, or social work within healthcare settings.
  • Preferred clinical licensure and experience in hospice or home care.
  • Valid driver's license and reliable transportation, with a clear driving record.
  • Willingness to travel locally and occasionally for overnight stays.
  • Strong verbal and written communication skills.
  • Ability to meet the physical demands of the role, including lifting and mobility tasks.
  • Collaborative mindset with a proactive approach to patient care.
  • Self-driven with a commitment to achieving goals and accountability.
