Position Overview:


Under the supervision of the Outpatient Clinical Manager, the Outreach Coordinator is responsible for delivering intensive outreach and engagement services to youth and their families, particularly in response to mental health or substance use crises.

The Outreach Coordinator collaborates with an interdisciplinary team to facilitate access to necessary psychiatric, medical, social, and financial resources tailored to the needs of the youth and their families.

This role involves networking with community partners to enhance the support system available to those we serve.

The Outreach Coordinator employs innovative strategies to engage clients in their communities, aiming to improve their overall health and well-being.


Key Responsibilities:

  • Conducts comprehensive assessments to develop personalized service plans that highlight the strengths and needs of clients, working closely with team members to implement these plans.
  • Engages in community outreach, including home visits, to build rapport with clients and facilitate their access to appropriate services.
  • Contributes to the creation of recovery-oriented care plans that address behavioral health, medical, and social challenges faced by clients, with a focus on crisis intervention and stabilization.
  • Maintains an extensive understanding of community resources to effectively connect clients with suitable services, ensuring clear communication with all involved parties.
  • Advocates for clients by employing creative strategies to help them identify and secure necessary resources and services.
  • Monitors service delivery through regular communication with clients and service providers to ensure continuity of care.
  • Completes all required documentation accurately and within specified timeframes, including follow-up communications as per program guidelines.
  • Assists clients in expanding their support networks beyond the behavioral health system.
  • Participates in weekly team meetings to discuss case strategies and engage in ongoing training related to outreach techniques and clinical practices.
  • Ensures compliance with all privacy regulations and quality assurance protocols, cooperating with review processes as needed.
  • Demonstrates proficiency in trauma-informed care, motivational interviewing, and the stages of change.
  • Effectively engages with individuals facing multiple challenges, including mental health issues, substance use, and other barriers.
  • Collaborates respectfully with internal and external partners to optimize service delivery for clients.
  • Fosters positive working relationships, contributing to a supportive team environment.

Qualifications:

  • Bachelor's Degree in a relevant social services field; a Master's degree is preferred (e.g., Social Work, Psychology, Counseling).
  • At least two years of experience working with youth experiencing severe emotional disturbances.
  • Certifications in CPR, First Aid, and Medication Administration; valid driver's license and insurance required.
  • Familiarity with community resources and mental health networks.
  • Proficient in providing clinical, outreach, and case management services.
  • Skilled in crisis resolution and de-escalation techniques.
  • Strong problem-solving abilities and innovative thinking to engage clients effectively.
  • Ability to navigate barriers to service access and engagement.
