ROLE DESCRIPTION:

The Community Outreach Specialist plays a crucial role in connecting community members with healthcare systems and social support resources to enhance their living conditions and health outcomes. This position assists individuals in navigating the complexities of physical health, mental health, and social services by addressing social determinants of health and acting as a liaison between clients, healthcare providers, and social service organizations. The role encompasses both clinical and community-based environments, including client residences.

KEY RESPONSIBILITIES:

  1. Conduct thorough assessments for assigned clients, providing appropriate referrals and following up on outcomes and next steps.
  2. Engage with clients through various communication methods (phone, text, email, home visits) to identify health service needs and schedule necessary appointments.
  3. Support clients in accessing and comprehending health information from their medical and social service providers.
  4. Assist clients in completing necessary paperwork related to health and social determinants of health, including applications and registration forms.
  5. Perform home visits as required to deliver education, follow-up, support, and screenings.
  6. Collaborate with internal and external partners to ensure an integrated approach to patient care, care coordination, and client engagement.
  7. Participate in community initiatives to promote services and encourage healthy behaviors.
  8. Maintain accurate and current client records by documenting services and interactions in the designated database system.
  9. Complete required training for community health workers and any additional assigned training.
  10. Execute other duties as assigned.

EDUCATION AND EXPERIENCE:

A high school diploma or GED is required, along with a valid driver's license, insured and reliable transportation, and lived experience or strong connections within the community. Experience in community outreach, peer counseling, health promotion, customer service, healthcare/medical office settings, and/or data entry is preferred.

COMPLIANCE STATEMENT:

Adhere to all applicable State and Federal laws and regulations.

PHYSICAL DEMANDS:

This position requires light physical activity associated with routine administrative tasks. The ability to lift up to thirty (30) pounds is necessary.

WORK SCHEDULE:

This role typically operates during standard daytime hours, Monday through Friday. Attendance at off-site meetings may be necessary, and local travel is required. A valid driver's license and insured vehicle are essential for this position.
