POSITION SUMMARY:
The Community Outreach Specialist is tasked with building connections between community members, healthcare systems, and social service resources to enhance health outcomes and life circumstances. This role assists individuals in navigating physical health, behavioral health, and social services by addressing social determinants of health and acting as a liaison among individuals, healthcare providers, and social service organizations. The position involves working in both clinical environments and community settings, including client residences.
KEY RESPONSIBILITIES:
- Conduct thorough assessments for assigned clients, provide appropriate referrals, and follow up on outcomes and next steps.
- Engage with clients through various communication methods (phone, text, email, home visits) to address health service needs and schedule necessary appointments.
- Support clients in accessing and comprehending health information from their medical and social service providers.
- Assist clients in completing necessary paperwork related to health and social service needs, including applications and registration forms.
- Perform home visits as required to deliver education, follow-up, support, and screenings.
- Collaborate with both internal and external partners to ensure integrated patient care, care coordination, and client engagement.
- Participate in community initiatives to promote services and encourage healthy behaviors.
- Maintain accurate and current client records by documenting services and interactions in the CHW database system.
- Complete community health worker training and any additional assigned training.
- Carry out other duties as assigned.

EDUCATION AND EXPERIENCE:
A high school diploma or GED is required, along with a valid driver's license, insured and reliable transportation; lived experience or strong connections within the community is essential. Experience in community outreach, peer counseling, health promotion, customer service, healthcare/medical office settings, and/or data entry is preferred.
COMPLIANCE STATEMENT:

MEDICAL HOME STATEMENT:
Understand the expectations and responsibilities associated with the Patient-Centered Medical Home model. Be aware of your role, complete relevant duties, and maintain the highest level of proficiency within your scope as part of a team-based approach to patient care.
PHYSICAL DEMANDS:
This position requires light physical activity associated with non-strenuous daily administrative tasks. The ability to lift up to thirty (30) pounds is necessary.
WORK SCHEDULE:
The standard work hours for this role are Monday through Friday during the day. Attendance at off-site meetings will be necessary, with local travel required. A valid driver's license and insured vehicle are mandatory. Occasional out-of-town travel may also be necessary.
