Your Specific Responsibilities:

Plan and coordinate age-friendly community efforts, including developing partnerships, programs and initiatives geared toward meeting the needs of aging individuals to maximize their health, safety and wellness.

Develop and deliver approved presentations for small and large groups on priority health topics, such as dementia, fall prevention, etc.

Partner with other county health departments and related health or social service agencies to gather data. Reports, tracks and shares data with managers and partners (as appropriate) to evaluate program or topic progress and opportunities. Provides quality assurance for program activities.

Serve as a community outreach liaison for the Department of Health, with the objective of improving public health outcomes for aging individuals, in alignment with the Community Health Improvement Plan.

Represents the Department of Health at community meetings and serves on councils, boards and task forces in a positive, professional manner. Actively follows program topics across state and national level for trends.

Adhere to all budget allocations by ordering and submitting purchasing orders for annual budgets and submitting reports as requested.

Other duties as assigned: to include shelter duty and participation in special departmental events.

Required Knowledge, Skills, and Abilities:

  • Knowledge of the principles and techniques of effective communication
  • Knowledge of the principles and practices of community health education.
  • Knowledge of basic public health marketing principles.
  • Skills in operating a personal computer.
  • Ability to develop and deliver health education and community development services to others.
  • Ability to communicate effectively - oral and written.
  • Ability to utilize Microsoft Office programs, including Work, Excel, PowerPoint, Outlook and Publisher.
  • Ability to design and produce health promotion and public health educational materials.
  • Ability to plan, organize, prioritize, and coordinate work assignments.
  • Ability to maintain accurate records.
  • Ability to work independently.
  • Ability to establish and maintain effective working relationships with community partners and others.
  • Ability to operate audiovisual equipment.
  • Ability to understand and apply applicable rules, regulations, policies, and procedures.
  • Ability to lift and carry equipment weighing 30 lbs. or more – unassisted.

Qualifications:

Minimum -

Experience teaching youth or adults

Experience working with community partners

Experience working in injury prevention

Preferred -

Bilingual

Bachelor degree in a related field.

Ability to translate professionally to clients
