The Social Worker Program Coordinator is responsible for overall program coordination, lo include, managing daily program operations; developing and implementing local policies and procedures based on national policies for Veterans al high risk for suicide; tracking and monitoring high risk patients; responding to inquiries at the local, VISN, and national level; preparing reports and statistics for facility, VISN and national use; and managing program budget.


The Social Worker Program Coordinator is responsible for conducting regular and routine outreach activities within the VA System and with Community Partners to provide information, education and create meaningful and sustainable partnerships to enhance the overall mission of the program.

Outreach activities include providing education and consultation/support to internal and external customers.

Duties include but are not limited to:

tracks and monitors high-risk patients to ensure that needed services are being received.

coordinates and manages clinical care for high-risk patient, when necessary, in accordance with local, VISN, and national guidelines and best practices.

receives referrals from the national Veterans Crisis Line NF/SGVHS providers/staff:

and other Suicide Prevention programs. The Social Worker Program Coordinator establishes contact with Veterans who need assistance to expedite access to mental health care.
independently initiate suicide prevention protocols for high risk Veterans when clinically indicated.

provides education and training to clinical and non-clinical staff in the NF/SGVHS about risk factors, warning signs, treatment options and resources for Veterans whom arc suicidal.

The Social Worker Program Coordinator provides similar training for community organizations that provide services to the Veteran population.

facilitates on-going training program for non-clinical front line staff to assure that they are aware of options for Veterans, whom they come in contact during crisis situations.

promotes the use or evidence based interventions and strategies for suicide prevention within NF/SG VHS.

Interfaces with services throughout the system, including, but not limited to, mental health providers and patient advocates, to resolve administrative and clinical problems.

This includes collaboration on the care of individual patients and the coordination of mental health care within the treatment team.

plans, designs, and implements the program, determines program goals and objectives, and completes reports for the VISN, the national program office, and/or VA Central Office.
