Position Overview:

The Student Health Care Center is seeking a dedicated Clinical Programs Coordinator II to join our dynamic team. In this role, you will be essential in managing and improving our case management system, ensuring our patients receive the highest quality of care. You will work collaboratively with healthcare providers to enhance continuity of care, follow-up, and referral processes.

Key Responsibilities:
  • Document case management services in medical records and assist students with their social service needs.
  • Facilitate the sharing of clinical information among care providers to ensure students receive appropriate services.
  • Conduct assessments and evaluate the effectiveness of services, ensuring patient satisfaction.
  • Provide crisis intervention and consultation to enhance student care.
  • Collaborate with external mental health programs and clinics for effective discharge planning and community referrals.
  • Support students through the academic withdrawal process, assessing their stability for services.
  • Collect and maintain accurate departmental statistical records and submit necessary reports.
  • Act as a liaison with community organizations to optimize student care.
  • Conduct ongoing evaluations and assessments, including patient satisfaction surveys.

Work Schedule:
Monday through Friday from 8:00 a.m. to 5:00 p.m. Flexibility may be required based on the needs of the Student Health Care Center.

Qualifications:
Applicants should possess a Bachelor’s degree in a relevant field and have at least two years of related experience. Preferred qualifications include a degree in social work or a related discipline, licensure in the State of Florida, and experience in college mental health.
