About the Role:

Job Summary

Under the general supervision of the Practice Manager, the Program Coordinator is responsible for overseeing and managing patient access, including all urgent and semi-urgent referrals to and from the practice and associated ancillary programs within NGHS and surrounding markets involved in patient care. This role is crucial in facilitating the journey of Hepato-Pancreato-Biliary patients through NGHS, focusing on evaluating systems-based practices and optimizing existing resources. The Program Coordinator will act as the primary clinical liaison and will also manage the HPB-NSQIP database, which includes the creation of scheduled and requested reports. The role involves assessing ongoing systems processes and developing quality improvement initiatives to enhance clinical care and resource utilization.

Minimum Job Qualifications

  • Licensure or other certifications: LPN
  • Educational Requirements: Associate's degree required. Four (4) additional years of clinical experience in a hospital or clinical practice setting may be considered in lieu of a degree.

Essential Tasks and Responsibilities

  • Builds strong relationships with key strategic referrers to enhance access and increase case volume.
  • Acts as the initial point of contact for patients, their families/representatives, external agencies, physicians, and referrers. Addresses inquiries regarding access to Hepato-Pancreato-Biliary care, typically received through Epic work queues and Online Inquiry.
  • Coordinates outpatient referrals and, when necessary, manages admission/scheduling for appropriate inpatient needs for NGPG General Surgery and Hepato-Pancreato-Biliary services.
  • Promotes NGPG Hepato-Pancreato-Biliary treatment options to internal clinics and external organizations through direct education to clinic staff, health fairs, community outreach events, and/or community activities.
  • Maintains and fosters working relationships with referral coordinators, both internal and external, to eliminate barriers or delays in scheduling consultations with the clinic.
  • Assesses and enhances practices for all clinical care within the Hepato-Pancreato-Biliary program. Functions in a primary nursing capacity, collaborating closely with the surgeon and PA during clinic and non-clinic days as required.
  • Collaborates with physician leadership and designees (e.g., resident physicians) in Hepato-Pancreato-Biliary Surgery to review and create quality improvement processes within the program. Coordinates with NGHS and NGMC leadership as necessary for quality enhancement within the program's scope.
  • Responsible for generating and evaluating data reports from the HPB-NSQIP database system.
  • Works closely with the surgery scheduler to optimize all surgical scheduling and patient management.
  • Coordinates with the administration of NGHS, NGPG, and the Cancer Center in program development and quality improvement initiatives.
  • Identifies trends with high-volume referring offices and collaborates with Business Development to continually foster positive community referral relationships.
  • Maintains Qgenda schedules for Surgeon and Resident Call Schedules as requested.
  • Orders supplies, identifies and enters IT ServiceNow tickets, and reports variances using RL6 system software.
  • Schedules surgeries/procedures as needed.
  • Other duties as assigned.
