Job Overview:

  • Contribute as an Intensive Case Management (ICM) Health Caseworker.
  • Address barriers to safety and wellness for vulnerable Afghan Humanitarian Parolees.
  • Provide client-centered case management for various refugee populations.
  • Collaborate with internal and external parties to develop service plans for individual client healthcare needs.

Key Responsibilities:

  • Identify healthcare needs for vulnerable clients.
  • Assist clients in navigating healthcare systems and scheduling appointments.
  • Advocate for clients in accessing healthcare services.
  • Develop and distribute healthcare guides tailored to each client.
  • Conduct outreach to address health needs and identify resources.
  • Provide orientation on health-related topics to clients.
  • Document services provided accurately and maintain client case files.
  • Attend relevant trainings and meetings.
  • May lead volunteers and interns' activities.
  • Promote team engagement and cohesion within IRC San Jose.
  • Support initiatives to strengthen safeguarding and adherence to IRC values.
  • Other duties as assigned.

Requirements:

  • Bachelor's degree in social work, public health, or related field preferred.
  • 1-2 years of case management experience required.
  • Demonstrated skills in relationship building, communication, problem-solving, and time management.
  • Fluency in English required; proficiency in Dari and Pashto desired.
  • Proficiency in MS Office applications and internet use.
  • Driver's license and access to a personal vehicle.

Working Environment:

  • Combination of office and field work within the service delivery area.
  • Occasional weekend and evening work may be required.
