As a key member of our team, you will provide intensive, therapeutic case management services to assigned individuals, working closely with clients to assess their health and support service needs, and develop personalized care plans to address their unique challenges.
Key Responsibilities- Case Management: Conduct initial and ongoing assessments of client needs, set goals, and develop written care plans with clients, identifying problems and needs, and outlining intended interventions and expected results.
- Therapeutic Interventions: Provide therapeutic support to clients, including individual counseling, support, and referrals to internal and community services, as needed.
- Documentation and Reporting: Maintain accurate and thorough documentation of client interactions, progress, and outcomes, and assist in preparing reports as required.
- Collaboration and Communication: Work effectively with clients, colleagues, and community partners to ensure seamless delivery of services and achieve desired outcomes.
- Education and Experience: Bachelor's degree in Social Science, Public Health, Healthcare Administration, or related field, with at least 3 years of experience providing human services to high-risk, medically underserved populations.
- Certifications and Training: Current American Heart Association healthcare provider CPR (BLS) certification, or equivalent, and ability to travel within San Diego County as needed.
- Skills and Abilities: Excellent interpersonal and customer service skills, basic computer literacy, and ability to work well in a team-based environment and independently.
