Job SummaryThe Outreach Case Manager will provide intensive case management services to clients, including conducting intake and individualized needs assessments, developing and implementing individualized service plans, and providing referrals and linkages to resources that support clients in achieving their goals.
Key Responsibilities- Conduct thorough intake and individualized needs assessments for all clients, identifying strengths and challenges, and developing individualized service plans that address housing goals, income, and other personal goals.
- Provide referrals, linkages, information, and support to resources that help clients achieve their service plan goals.
- Evaluate client progress and adjust service plans as needed.
- Maintain accurate and up-to-date client records, including case file notes and documentation.
- Develop and maintain relationships with community partners and stakeholders to enhance efforts to engage and retain individuals in housing goals.
- Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations.
- Strong analytical and problem-solving skills, with the ability to think critically and make sound decisions.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Valid driver's license and reliable transportation.
- Bachelor's degree in a related field, such as social work, psychology, or counseling.
- Experience working with homeless populations, including those with complex needs.
- Knowledge of community resources and services available to support individuals experiencing homelessness.
