POSITION SUMMARY
Responsible for providing and coordinating resident care to ensure quality delivery and leading the care team effectively.



ESSENTIAL FUNCTIONS

  • Ensure residents receive care with kindness and respect
  • Prevent accidents and injuries for residents
  • Take charge in ensuring resident safety during emergencies
  • Act as a resource person for team members
  • Participate in orientation and training for care team members
  • Foster a positive work environment and support the development of skills among the care team
  • Review and update resident care plans
  • Communicate physician orders to staff and family members
  • Assist with schedule coverage as needed
  • Administer medications according to policies
  • Conduct resident evaluations and provide necessary support
  • Maintain medication and supply inventory
  • Monitor and report resident behavior changes
  • Complete necessary documentation and reporting
  • Participate in activities and ensure resident participation
  • Adhere to safety, infection control, and confidentiality policies
  • Partake in on-call rotation
  • Build positive relationships with residents, staff, and visitors
  • Remain alert during duty hours
  • Fulfill any additional duties required

SKILLS AND ABILITIES

  • High school diploma/GED and completion of Care Specialist I and II curriculum within 90 days of employment
  • Minimum 6 months of recent experience as a Care Specialist or equivalent
  • CPR/First Aid certification required
  • Proficiency in Microsoft Office and willingness to learn company-specific software
  • Strong communication skills
  • Excellent problem-solving and organizational abilities
  • Team-oriented and able to work with minimal supervision
  • Ability to maintain confidentiality and professionalism


PHYSICAL DEMANDS

  • Stand, walk, bend, twist frequently
  • Ability to lift and carry varying weights
  • Manual dexterity for tasks as required
