We are seeking a skilled Speech Language Pathologist to join our Home Health team at CenterWell Primary Care. As a key member of our care team, you will play a vital role in providing high-quality patient care and support.
Key Responsibilities- Evaluate and provide speech/language pathology services to patients in their homes or facilities
- Develop and implement individualized treatment plans to address patients' communication and swallowing needs
- Collaborate with healthcare professionals to ensure comprehensive care and coordination
- Conduct assessments and provide therapy to patients with a range of communication and swallowing disorders
- Document patient progress and communicate with healthcare teams to ensure seamless care
- Participate in care coordination activities and discharge planning to ensure smooth transitions for patients
- Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
- Minimum of six months experience as a speech therapist/speech language pathologist
- Home Health experience a plus
- Current and unrestricted license
- Current CPR certification
- Good organizational and communication skills
- A valid driver's license, auto insurance, and reliable transportation are required
- A competitive salary range of $76,800 - $105,800 per year
