About the Role

We are seeking a skilled Speech Language Pathologist to join our Home Health team at CenterWell Primary Care. As a Speech Language Pathologist, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Key Responsibilities
  • Evaluate and provide speech/language pathology services to patients in their homes or facilities, ensuring they receive the best possible care and support.
  • Develop and review treatment plans in collaboration with patients, families, and healthcare teams, ensuring that each patient's unique needs are met.
  • Utilize your expertise to assess and treat disorders of speech, voice, language, hearing, and swallowing, minimizing their impact on patients' lives.
  • Administer and interpret diagnostic tests and therapeutic treatments, including audio logic screening, to inform your treatment decisions.
  • Monitor patients' progress, reporting changes and updates to supervisors and physicians, ensuring seamless communication and care coordination.
  • Provide education and training to patients and their families on alternative communication systems, empowering them to take control of their care.
  • Offer guidance and support to patients, families, and healthcare staff, fostering a collaborative and patient-centered approach to care.
  • Maintain accurate and timely documentation, submitting reports and summaries as required by Company policy.
  • Participate in care coordination activities and discharge planning, ensuring a smooth transition for patients and their families.
  • Adhere to the highest standards of professional conduct, maintaining confidentiality and sharing information only when necessary and in the best interests of patients.
  • Engage in ongoing professional development, staying up-to-date with industry best practices and regulatory requirements.
Requirements
  • Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
  • Minimum of six months of experience as a speech therapist/speech language pathologist.
  • Home Health experience is a plus.
  • Current and unrestricted license.
  • Current CPR certification.
  • Excellent organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation are required.
