The Senior Housing Supportive Services Specialist will coordinate the AIDS Foundation of Chicago (AFC) supportive housing programs funded by the U.S. Department of Housing and Urban Development (HUD), including HUD's Housing Opportunities for People with AIDS (HOPWA) programs, state and city government agencies, and private foundations. This position also serves as AFC's liaison to housing program sub-contracted partner agencies that provide housing and direct services for chronically medically ill (including HIV/AIDS), chronically homeless, and low-income individuals and families. This position also provides guidance and direction to partner agency direct service staff and supervisors about the implementation of the housing program policies and procedures and has shared responsibility for the overall quality of services provided.

The salary range for this role is $50,000 to $52,000 annually. 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Program Coordination

• Provide support to case managers and supervisors at various funded partner agencies to provide high quality services

• Provide oversight and direction to supervisors and case managers from partner agencies in accordance with supportive housing program policies and

procedures

• Utilize Coordinated Entry System (CES) to track project data and receive project referrals; initiate the referral process to ensure that it is expedited to assess for eligibility, and review and approve eligibility documentation for potential program participants

• Monitor utilization of support services (i.e., case management services, transportation, furnishings, dental, vision); provide information and reports on a regular basis on program outcomes, service coordination, and service delivery to AFC staff and partner agency program directors

• Collaborate with SHP Director & SHP Senior Manager to update SHP policies and procedures and to ensure alignment with current program operations and system-level changes

• Serve as community point of contact and share information for all inquiries about SHP projects

• Collaborate with AFC's Senior Quality Specialist; provide ongoing technical assistance to partner agency service providers in use of case management database (i.e., Case Worthy)

• Serve as main point of contact for external stakeholders within the Chicagoland Homeless Services System

• Lead short-term projects as needed (i.e., system-level changes to policies and procedures)

Quality Assurance and Data Entry

• Ensure accuracy and timeliness of intake data by entering intakes and exits data on all program participants in data tracking systems (i.e., Case Worthy &

HMIS)

• Ensure accurate entry of intake & exit information by partner agencies for accurate reporting

• Communicate with partner agency case managers and supervisors when quality issues arise

• Maintain compliance with all HMIS Data Standards in accordance with HUD CoC requirements by updating referral status when clients are enrolled; complete housing move-in date and exit assessment within 7 days of discharge

• Complete monthly reports for assigned programs

• Participate in annual program partner site visit reviews

• Support SHP Director and Senior Manager in chart audits for Funders

• In conjunction with Program Development & Senior Quality Specialist, monitor data to ensure programs are meeting quarterly and annual Data Quality

Standards and the Annual Data Evaluation Standards to ensure programs are maintaining competitiveness with other city-wide CoC Funded Projects

• Field participant calls and complaints as necessary.

Meetings and Training Facilitation

• Convene and facilitate System Integration Team (SIT) meetings with partner agency service providers on a monthly basis for multiple housing programs

• Provide guidance or case consultation as need to partner for the development of client-level interventions

• Attend all Department & Inter-departmental meetings (i.e., Payment Processing, Public Grants Prep, SHP Team meeting, Quality Assurance Team meeting & All Housing Team)

• Attend and participate in systems-level meetings and workgroups (i.e., Continuum of Care (CoC), Chicago Low Income Trust Fund (CLIHTF), Coordinated Entry System (CES)

• On-board new partner agencies' case managers and supervisors on Supportive Housing processes, expectations and policies and procedures

Other

• Assist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others

• Update job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in

professional organizations

• Protect organization's value and manage risk by keeping information confidential

• Perform other duties as assigned

ENTRY REQUIREMENTS (EXPERIENCE AND EDUCATION)

Minimum Qualifications

Bachelor's degree and 2 or more years of Human Services experience OR 5 or more years Human Services experience

Preferred Qualifications

Master's degree in Psychology, Social Work, Counseling or related field 1 or more years Case Management experience

1 or more years Homeless Services experience

1 or more years HIV-specific service experience

HUD Knowledge:
Possesses knowledge of the U.S. Department of Housing and Urban Development (HUD) programs, requirements, rules, and procedures, essential for coordinating supportive housing programs

Interpersonal Skills:
Develops and maintains professional, trusting, and positive working relationships with managers, supervisors, staff, co-workers, partner agencies, and vendors. Engages others in conversation and actively participates while interacting with empathy and compassion

Supportive Housing:
Understands supportive housing programs for individuals/families facing homelessness and serious challenges such as addiction, mental health issues, and HIV/AIDS. Able to implement a Housing First model effectively
