Bilingual Housing Support Specialists for the Chicagoland Area
- Deliver comprehensive home-based support services to clients and their families, focusing on stabilization in permanent housing, enhancement of income, and skill development.
- Conduct intake interviews, assessments, service planning, and annual progress evaluations.
- Facilitate life skills training in both formal and informal environments, covering areas such as financial literacy, tenant rights, parenting skills, and health/wellness education.
- Collaboratively create and revise individualized service plans with clients, offering motivation, education, and advocacy to help achieve their personal objectives.
- Assist in mediation for conflicts arising between clients and landlords, family members, or other involved parties.
- Provide crisis intervention support as necessary.
- Connect clients with community resources and manage referrals for identified services in progress notes or service plans.
- Transport clients in a personal vehicle when appropriate.
- Coordinate with team members, including Property Advocates, Health Services Managers, and Early Childhood Specialists as required.
- Support clients in locating and transitioning to new housing.
- Offer crisis intervention for residents during evenings and weekends.

Qualifications:
Bachelor's degree required.
Minimum of 1 year of experience in Case Management.
Must possess a valid Driver's License.
Vehicle is required for this position.
