About Chloe House:

Chloe House serves as a Pre and Post-Transitional Care Facility for adolescent girls referred by various youth services. Our mission focuses on creating a nurturing environment that promotes collaboration with community partners to achieve successful outcomes for our residents. We offer a range of services including counseling, life skills training, and educational support, alongside family counseling aimed at enhancing personal development and self-expression. Our commitment extends to facilitating reunification efforts and providing access to essential resources during the youth's stay.

Key Responsibilities:

  • Exhibit leadership and provide support to staff, particularly in the absence of the Program Manager.
  • Take initiative in fostering a structured program environment for all youth participants.
  • Empower youth and families through proactive engagement and support.
  • Ensure effective supervision and safety of clients, maintaining cleanliness and adherence to house rules.
  • Oversee client assessments and case management services, guiding the development of individualized treatment plans.
  • Deliver timely and accurate documentation of services, including progress reports and urgent issues requiring attention.
  • Assist clients with discharge planning and aftercare strategies as they transition from the program.
  • Maintain awareness of local resources and services to address client needs, facilitating interagency referrals as necessary.
  • Engage actively in weekly staff meetings and maintain regular supervision sessions with the Program Manager.
  • Participate in live supervision sessions with clients and their families.
  • Attend all required training and in-service sessions as directed by the Program Manager or Clinical Director.
  • Ensure that client files and service plans are accurate, updated, and securely maintained.
  • Perform additional duties as assigned.

Qualifications:

  • Bachelor's Degree required; Master's Degree preferred or currently pursuing advanced education in Social Work, Case Management, Counseling, or a related field.
  • A minimum of 5 years of experience in the human services sector, particularly with adolescents and young adults.
  • In-depth understanding of homelessness-related issues.
  • Proven ability to collaborate with community leaders to secure program funding and effectively manage complex projects.
  • Strong capability to partner with individuals and families in challenging situations, empowering them to overcome obstacles.
  • Familiarity with the local DC community and available resources for youth and families.
  • Demonstrated personal responsibility, flexibility, and effective communication skills with diverse populations.
