Position Summary

The Case Manager plays a crucial role in delivering individualized case management services to young individuals residing in KBEC's Youth Transitional Housing Program. The primary objective is to assist clients in overcoming obstacles to secure and sustain permanent housing through a strengths-based, Housing First methodology. The Case Manager will provide on-site support and guidance.

Key Responsibilities
  • Deliver comprehensive, client-focused case management to youth in the Transitional Living Program.
  • Offer supportive services to individuals facing homelessness.
  • Conduct face-to-face interactions with clients, dedicating approximately 80% of time to direct engagement.
  • Exhibit empathy and adaptability in addressing the unique needs and requests of clients, promoting wellness and independence.
  • Engage clients in assessing their individual needs, preferences, and strengths.
  • Formulate, execute, and evaluate personalized housing case plans with clients to achieve goals related to health, financial management, social support, and daily living skills.
  • Provide financial literacy and budgeting assistance to help clients manage housing costs and daily expenses.
  • Establish trusting relationships with clients, assess their safety and well-being, and initiate emergency services when necessary.
  • Oversee medication management for clients as required.
  • Complete all necessary documentation and case notes in the Homeless Management Information System (HMIS).
  • Participate in Coordinated Entry Assessment by conducting required SPDAT assessments.
  • Deliver person-centered supportive services through the Youth Transitional Living program as needed.
  • Contribute to the planning and development of new initiatives related to client services.
  • Maintain connections with external agencies that offer services to homeless individuals.
  • Engage actively in case meetings, staff discussions, and supervision sessions.
  • Participate in training and professional development opportunities as requested.
  • Assist with volunteer coordination as necessary.
  • Perform additional duties as assigned.
  • Develop and revise individual housing and service plans.
  • Support clients in locating and selecting wraparound services.
  • Assist clients in securing housing resources, including tracking applications and lease agreements.
  • Act as a liaison with landlords and community stakeholders.
  • Attend relevant professional training and conferences.
Qualifications
  • Bachelor's degree in psychology, social work, humanities, social science, or a related field, particularly in youth services.
  • At least two years of relevant experience working with youth, especially those facing homelessness or other societal challenges.
  • In lieu of a degree, candidates may qualify with five years of direct client work experience in social or human services.
  • Ability to submit documentation within a 24-hour timeframe.
  • Experience providing counseling or case management to vulnerable youth populations.
  • Familiarity with domestic violence victim support is preferred.
Skills and Competencies
  • Proficient in using computer software for data entry and management, including HMIS.
  • Skilled in Microsoft Office Suite and Google Suite.
  • Strong written and verbal communication abilities.
  • Exceptional organizational skills with the capacity to manage multiple tasks effectively.
  • Ability to maintain confidentiality and demonstrate professionalism.
  • Commitment to social justice and equity across various dimensions.
  • Capability to develop individualized housing stabilization plans with targeted goals.
  • Experience in the DC Homeless Service arena, including conducting assessments.
Working Conditions

This position involves a mix of office work and travel to meet with clients and attend meetings with stakeholders.

Physical Requirements

Employees must be able to sit for extended periods and may need to lift up to 50 lbs. A valid driver's license is required for travel.

Background Checks

Candidates must pass a criminal history background check, including fingerprinting and various registry checks.

Salary: $26.00 per hour
