About Chloe House:

Chloe House serves as a Pre and Post-Transitional Care Facility for adolescent girls referred by relevant social services. Our mission is to cultivate a nurturing environment while building strong partnerships within the community to ensure positive outcomes for our residents. At Chloe House, we offer counseling, life skills training, and educational and medical support. We also provide family counseling and empower youth in areas of personal development and self-expression. Our facility emphasizes reunification efforts and grants access to vital resources during the youths' temporary stay. Ultimately, our objective is to deliver holistic care that enables residents to take meaningful steps toward rehabilitation.

Key Responsibilities:

  • Exhibit leadership and provide support to staff, particularly in the absence of the Program Manager.
  • Assume a leadership role in upholding program structure and supporting all youth involved.
  • Proactively empower youth and families through effective support and guidance.
  • Ensure diligent supervision and maintain safety standards for clients, enforcing house rules and monitoring cleanliness.
  • Oversee client assessments, deliver case management services, and guide the development of personalized treatment plans.
  • Provide the Program Manager with timely and precise documentation of services rendered, including client progress and urgent issues.
  • Assist clients with discharge and aftercare planning as they transition to subsequent stages post-program.
  • Maintain awareness of local services that may serve as resources, assessing and identifying client needs for interagency referrals and follow-ups.
  • Engage actively in weekly staff meetings, demonstrating leadership and collaboration.
  • Meet regularly with the Program Manager for supervision and guidance.
  • Participate in live supervision sessions with clients and their families.
  • Engage in all SBY training and in-services, along with any additional training as identified by the Program Manager or Clinical Director.
  • Ensure that client files, records, monthly reviews, and Individual Service Plans are accurate, updated, and secure.
  • Perform other duties as assigned.

Qualifications:

  • Bachelor's Degree required; Master's Degree preferred or currently pursuing advanced education in Social Work, Case Management, Counseling, or a related Human Services field.
  • A minimum of 5 years of experience in the human services sector, preferably with adolescents and/or young adults.
  • Comprehensive understanding of homelessness-related issues.
  • Proven ability to collaborate with community leaders to secure program funding and lead complex projects effectively.
  • Demonstrated capacity to partner with individuals and families in challenging situations, empowering them to navigate their challenges successfully.
  • Strong familiarity with the local DC community and available resources, particularly those aimed at young people and their families.
  • Possess a strong sense of personal accountability, be driven, adaptable, and capable of effective communication with diverse or hard-to-engage populations.
