Overview:
The Case Manager plays a crucial role in delivering individualized case management services for participants in the Youth Transitional Housing Program at KBEC Group Inc. The primary goal is to assist individuals in navigating obstacles to secure permanent housing through a strengths-based, Housing First methodology. The Case Manager will provide direct, on-site support to clients.

Key Responsibilities:

  • Deliver thorough, client-focused case management to residents of the Youth Transitional Living Program.
  • Offer essential support services to individuals facing homelessness.
  • Engage with clients to evaluate their needs, preferences, and strengths.
  • Create personalized housing case plans with clients that encompass various objectives.
  • Support clients in budgeting and enhancing financial literacy.
  • Build trusting relationships with clients to ensure their well-being.
  • Oversee medication management for clients when required.
  • Accurately complete all necessary documentation and case notes.
  • Collaborate with external organizations that provide services to homeless individuals.
  • Participate in assessments and case discussions.
  • Attend professional development training and conferences.
  • Assist in identifying wraparound services and suitable housing resources.
  • Act as a liaison with landlords and community partners.

Qualifications:

  • Bachelor's degree in psychology, social work, humanities, or a related discipline.
  • 1-2 years of relevant experience working with youth in challenging circumstances.
  • Proficient in computer software applications and Microsoft Office Suite.
  • Familiarity with social service providers and public assistance programs.
  • Strong communication, organizational, and case management capabilities.
  • Ability to manage crisis situations effectively.
  • Dedication to equity and social justice principles.
  • Knowledge of homeless services and community resources.

Working Environment:
This position involves office work, travel to meet clients, and interaction with various stakeholders.

Physical Demands:
The role requires prolonged periods of sitting, operation of a motor vehicle, and occasional lifting.

Compensation: $26.00 per hour
