Job Description

We are seeking a skilled and compassionate Pediatric Home Health Nurse to join our team at NortheastArc-Clinical Services. As a member of our clinical staff, you will provide high-quality, patient-centered care to medically fragile pediatric and adult populations in their homes, schools, or community settings.

Key Responsibilities:
  • Provide one-on-one nursing care to patients with complex medical conditions, including feeding challenges, tracheostomies, ventilator support, seizure disorders, and more.
  • Develop and implement individualized care plans to meet the unique needs of each patient and their family.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Conduct thorough assessments, including respiratory and GI evaluations, to identify patient needs and develop effective care strategies.
  • Administer medications, perform wound care, and provide other medical interventions as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements:
  • Valid RN or LPN license in the state of Massachusetts.
  • Experience in pediatric home health care or a related field.
  • Strong clinical skills, including critical thinking and problem-solving.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Compassion, empathy, and a commitment to providing high-quality care.
