Job Description

Overview: As a Case Manager with Greater Lynn Senior Services, you will play a vital role in ensuring that older adults and individuals with disabilities have access to the services they need to live independently.

Responsibilities:

  • Conduct in-home assessments to determine the needs of consumers and create personalized care plans.
  • Identify, coordinate, authorize, and secure community-based services to meet the needs of consumers.
  • Modify and adjust care plans as the consumer's needs or situation changes.

Qualifications:

  • Experience in the human services field or a Bachelor's degree is desired.
  • Bilingual Spanish is required.
  • Excellent time management and computer skills are essential.
  • A strong desire to help others and establish positive relationships is necessary.
  • The ability to handle confidential information and maintain a professional demeanor is required.
  • Reliable transportation is necessary for home visits throughout the Greater Lynn Senior Services service area.
