Position Overview:
As a Clinical Care Supervisor, you will play a vital role in our mission at Kenneth Young Center. This position offers a unique opportunity to make a significant impact on the lives of individuals transitioning from nursing facilities in the Northwest Suburbs and Lake County.

Our team collaborates with volunteers, community partners, and those we serve to provide comprehensive behavioral health programs, services for older adults, and community health initiatives.


Key Responsibilities:
  • Provide clinical oversight and case management for Colbert and Williams class members.
  • Conduct assessments and coordinate care to address both mental and physical health needs.
  • Deliver treatment services in flexible settings that best meet clients' needs, including hospitals, nursing homes, and community locations.
  • Regularly meet with Class Members at designated facilities to ensure continuity of care.
  • Oversee and coordinate all service activities for class members during their transition.
  • Facilitate referrals and intake into mental health services.
  • Conduct comprehensive assessments that encompass all aspects of clients' lives, including emotional health, physical wellness, and social relationships.
  • Create individualized wraparound service plans with input from Class Members and their support networks.
  • Provide direct services, including skill-building for daily living activities and trauma-informed treatment.
  • Monitor post-transition progress for up to 18 months to ensure access to necessary services.
  • Build and maintain relationships with nursing facility staff to enhance program effectiveness.
  • Participate in training and development opportunities to further professional growth.

Qualifications:

A Master's Degree in Social Work, Psychology, Counseling, or a related field is preferred. A Bachelor's degree with substantial clinical experience may also be considered. Candidates should have:

  • At least 2 years of experience working with adults facing serious mental health challenges or complex health conditions.
  • Experience in non-traditional treatment environments, such as residential or community settings.
  • Proficiency in basic computer skills, including knowledge of Microsoft Office and Electronic Health Record systems.
