We are seeking a skilled Nurse Care Coordinator to join our team at Ascension. As a key member of our healthcare team, you will play a vital role in coordinating patient care activities and resources.
Key Responsibilities- Coordinate Patient Intake: Evaluate the severity of illness for all elective, direct, and urgent admissions to determine the appropriate level of patient care.
- Review Physician Orders: Review physician orders for clinical appropriateness, completeness, and proper admission status.
- Coordinate Education and Development: Coordinate education and development of internal staff and external departments to ensure understanding of intake review and approval process.
- Conduct Complex Clinical Review: Conduct complex clinical review for inpatient, outpatient, and ancillary services requests for medical necessity.
- Licensure/Certification/Registration: BLS Provider obtained within 1 month of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse Credentialed: Registered Nurse credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.
- Previous Hospice Experience: Minimum of 1 year previous hospice experience.
- BSN: BSN degree preferred.
