Job Title: Care Coordinator

At Sheppard Pratt, we are seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in providing recovery planning support to Adolescent Clubhouse members and case management services to their families.

Key Responsibilities:
  • Screen and Introduce New Members: Welcome new members and their families to the Clubhouse, ensuring a smooth transition and setting the stage for a successful recovery journey.
  • Develop Personal Goals and Family Participation Plans: Collaborate with members and their families to create personalized goals and plans, guiding involvement in Clubhouse activities and promoting recovery from substance abuse addiction.
  • Provide Case Management Services: Assess clients' situations, provide basic case management services, and connect them with relevant community resources, including entitlements and social services.
  • Ensure Service Delivery: Make follow-up contacts with clients and other agencies to guarantee that needed services are provided, and intervene in crisis situations as needed.
  • Consult and Collaborate: Work with county, community, and agency partners to identify and develop resources for client assistance, ensuring a comprehensive support system.
  • Support Senior Peer Leadership Specialist: Assist the Senior Peer Leadership Specialist with activities as needed, promoting a cohesive and supportive team environment.
  • Document and Record Keeping: Review programmatic and personal documents, create and maintain case contact notes and records, and perform related functions.
  • Transportation Services: Provide transportation to clients as needed, utilizing company-owned vehicles and maintaining a valid state driver's license with a clean driving record.
Requirements:
  • Experience: Requires two years of experience in casework, guidance, or counseling, and peer navigation.
  • Education: Requires a Bachelor of Arts in a Human Services-related field; a Master's degree is preferred.
  • Communication and Interpersonal Skills: Ability to communicate effectively with parents, community professionals, and deal with a variety of personalities and situations with youth, staff, and parents.
  • Sensitivity and Cultural Competence: Sensitivity to substance abuse issues among teens and their families, with a commitment to cultural competence and understanding.
  • Transportation and Driving Record: May provide transportation in a company-owned vehicle; must possess a valid state driver's license with a clean driving record.
Working Conditions:
  • Work Environment: Generally, work occurs in a programmatic environment with no physical discomfort due to temperature, dust, or noise; noise levels may fluctuate.
  • Flexibility and Travel: Flexibility in working hours may be needed to coordinate direct care and services with families and/or to provide outreach; depending on assignment, work requires travel between programs and services.
