Compensation & Benefits:
This is a full-time, salaried position offering a compensation range of $50,000 to $55,000 annually.

Key Responsibilities:
  • Act as the primary liaison for clients, caregivers, and healthcare professionals throughout the care process.
  • Collaborate with the team to create and execute personalized care plans for each client.
  • Advocate for clients, ensuring their needs and preferences are prioritized.
  • Conduct intake assessments and regularly review and adjust client care requirements.
  • Organize and coordinate client appointments, promptly communicating any changes.
  • Maintain accurate documentation and records in accordance with company policies.
  • Work alongside caregivers, clients, and healthcare professionals to resolve any issues that may arise.
  • Provide resources and support to clients and caregivers to enhance care outcomes.
  • Engage in team meetings, offering insights and feedback to refine care processes.

Qualifications:
  • Bachelor's degree in healthcare administration, social work, or a related discipline.
  • At least 2 years of experience in home care coordination or a similar role.
  • Familiarity with healthcare regulations and best practices.
  • Outstanding communication and interpersonal skills.
  • Strong organizational and time management capabilities.
  • Proficient in Microsoft Office and electronic medical records systems.
  • A compassionate and patient-centered approach.
