Key Responsibilities:- Conduct thorough health assessments and develop personalized care plans for patients
- Order and interpret diagnostic tests, prescribe medications, and provide treatment as needed
- Collaborate with other healthcare professionals to coordinate patient care and ensure seamless transitions
- Stay up-to-date with the latest medical research, guidelines, and best practices in family medicine
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal patient outcomes
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree
- Current licensure as a Registered Nurse (RN) and Nurse Practitioner (NP) certification
- Experience in family medicine or primary care
- Strong clinical skills and ability to work independently
- Excellent communication and interpersonal skills
