About the Role

CN Guidance and Counseling Services is seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing care coordination to adults diagnosed with severe mental illness, multiple medical co-morbidities, and/or co-occurring substance abuse disorders.

Key Responsibilities
  • Conduct Initial and Ongoing Assessments: Conduct thorough assessments of assigned clients to document strengths, needs, goals, and resources.
  • Develop and Implement Care Plans: Participate in the development and implementation of client-centered comprehensive, integrated, interdisciplinary care plans in consultation with the Program Supervisor and other team members.
  • Communicate Effectively: Maintain effective communication with clients, primary care physicians, substance abuse and mental healthcare providers, family, collateral resources, and other Agency staff on behalf of clients.
  • Document and Record: Maintain accurate and timely documentation of client records, statistics, and other related reports in accordance with policy and procedure.
  • Coordinate Care Planning: Coordinate care planning with other providers of services/resources to ensure goal-directed, collaborative care, including care transitions.
  • Participate in Team Meetings: Participate in team meetings to provide input/feedback around psychosocial conditions/comorbidities to review client status, update plans and goals, and review outcomes to further program goals.
  • Provide Outreach and Engagement: Provide telephonic and face-to-face outreach, engagement, and service planning in the field.
  • Link Clients to Services: Act as a linkage to community services, including medical, behavioral, residential, entitlement, and other needed services per interdisciplinary care plan.
  • Monitor Service Delivery: Monitor overall service delivery to clients to ensure coordination and continuity; advocate with service providers/resources as needed.
  • Provide Crisis Intervention: Provide crisis intervention and follow-up on an as-needed basis 24 hours a day, 7 days a week.
  • Participate in Supervision and Training: Participate in ongoing supervision, training, and education as needed to ensure a high quality of service delivery and continued professional growth.
Requirements
  • Education and Experience: For H.S. Diploma/GED level candidates, four (4) years of related human service and direct service experience required. For B.A. level candidates, one (1) year of related human service and direct service experience required.
  • Preferred Qualifications: Bachelor's Degree or Master's Degree in one of the following fields: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy.
  • Driver's License: A valid Tri-state (New York, New Jersey, or Connecticut) driver's license is required.
